What is the difference between a Safety Data Sheet and a transport document?

The SDS is a product document — prepared by the manufacturer for the substance itself and covering all its properties, hazards, and regulated uses. It does not change with each individual shipment. The transport document is a consignment document — prepared by the shipper (consignor) for each specific ADR movement, declaring the quantity packed, packaging type, collection address, and delivery address for that journey. Section 14 of the SDS is the source from which the shipper extracts the UN number, proper shipping name, hazard class, and packing group to fill in the transport document. Both documents are mandatory on every ADR road freight movement. The SDS travels as a product reference; the transport document travels as the shipment declaration. Neither replaces the other.
